Description

The American Medical Association Foundation (AMAF) announces the availability of funds through our Community Health Grants (CHG) program. AMAF seeks to financially support innovative programs targeting the prevention and reduction of type 2 diabetes and hypertension. Grant funding will support community-based organizations such as free health clinics that provide services to vulnerable and underserved populations.  The deadline for all applications is Wednesday, December 20, 2017 at 11:59pm CST.*

The AMA Foundation aims to increase health equity and improve health outcomes among vulnerable populations utilizing evidence-based models of care. Committed to fostering focused and measurable health improvements, Community Health Grants will be awarded in amounts up to $60K and are renewable for up to three years.

This is a competitive process open to all community based organizations including free clinics that meet the following criteria:


  • U.S. based Non-Profit Organization with 501(c)3 status

  • In operation for over 3 years

  • Annual operating budget less than $3 million (if agency has multiple sites, the location that where the project will be implemented must have an annual budget LESS THAN $3 Million Dollars.)

  • Provide services in the continental U.S. 

Eligible respondents must also be able to demonstrate the following:

  1. Capacity to implement evidence-based and/or innovative prevention and intervention programming that has the ability to be replicated over time

  2. Document the ability to implement and sustain all aspects of a program (including outcome measurements/reporting) beyond the grant term

  3. Demonstrate innovative collaborations with other organizations (hospitals, social agencies, food banks, fitness centers, etc.) and clearly define the roles of each entity

  4. Predominantly serve an economically disadvantaged and/or underserved population (inner city and/or rural)

  5. Capacity to collect data to address the performance metrics outlined in this RFP

Previous AMAF grant recipients are eligible to submit provided that eligibility criteria are met and have fulfilled the terms and agreements for previous grants.

Two Pre-Submittal Webinars will be provided for interested organizations. These webinars will offer a comprehensive overview of the Community Health Grants program and review the RFP timeline. AMAF Program staff will also address any questions and concerns presented by webinar participants. 


The Pre-Submittal Webinars will be:

Monday, December 4, 2017         10:30AM – 12:00PM CST

Attendance is not mandatory but is strongly advised. Interested respondents need only register for one webinar as they will contain the same content. Those unable to attend either opportunity will be able to access the completed webinar presentation via the AMAF website after the first webinar has been completed.
